New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.
You will need to complete both the New Patient Registration Form and Registration Form (GMS1) The questionnaire in the New Registration Form will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Aged 40+: Take the Health Check
For your own safety, all new patients aged 40+ years joining the practice are requested to have a health check which is carried out by our practice nurse. This is a quick check which takes about 20 minutes and involves simple tests such as blood pressure and urine tests.
Medical treatment is available from 5 working days from the date of registration. Please contact reception for further information.
